Spanish Coalition for Housing's Health & Housing Initiative connects Chicago families to housing stability and preventive healthcare — addressing physical, mental, and financial health — in partnership with Rush, Wellness West, and the Center for Housing & Health. Our Community Health Workers, HUD Certified Housing Counselors and Housing Case Managers bridge hospital and home, so families get coordinated, wraparound support every step of the way.
